Chronic Disease Management, Complex Care:

New Patient Base Price of $120,  Existing Patient $90

Behavioral Health Counseling:

$90/session

Wellness & Physicals:

All Kid Physicals: $35

All Adult Physicals: $90

Procedures:

Pulmonary Function Test (PFT): $50

Cryotherapy for Skin Tag / Wart Removal: $15/each

Cyst Removal: $50-200 depending on size and location

Mole Removal / Shave: $40/each, plus cost of diagnostic lab if requested

Ear Wax Removal (lavage/curette): $25-50 depending on severity/complexity

Injections:

Administration Assistance with Injectable Medications (does not include medication cost): $15

Trigger Point Injections (includes administration and medication): $40-100 / each depending on size and location

Joint Injections (includes administration and medication): $50-75 / each depending on location

Tests:

Rapid COVID-19 Antigen Test (Swab): $50

Rapid Flu A/B Test (Swab): $30

Rapid Strep Test (Swab): $30

Drug Screen (Urine): $30

Pregnancy Test (Urine): $15

Urinalysis (Urine Dip): $15

Glucose (Blood): $15

TB Testing (PPD Skin Test):  Placement: $35, Reading: $25

Vaccines:

Flu Vaccine: $40 (Available starting October 22nd, 2022)
